Exemption Statement

By checking this box and typing my name below, the undersigned does hereby certify under penalty of perjury that: This is to certify that the officers and employees selected above will make no expenditures reportable under Chapter 2, Article XXX of the Code of the City of Phoenix; therefore, shall be exempt from filing quarterly expenditure reports. If subsequently anyone does make a reportable expenditure, within 10 days that officer or employee shall notify the City Clerk in writing of such expenditure and thereafter conform to the reporting requirements.
